Turning 18 - Transitioning from Children's to Adult Services

Children receiving Intellectual and Developmental Disability (I/DD) Services within Washington County will be transitioned to Adult I/DD services upon turning 18. Your current Children’s Services Coordinator will help you plan and prepare for the changes in services and to coordinate a smooth transition to your newly assigned Adult Services Coordinator. Community Group Information:

  • FACT Oregon – Transition to Adulthood - Provides information and action steps that can be taken during the ages of 16-18. Includes information about diploma requirements, modified diplomas, pre-employment transition services, and more.

Brokerage Services Choice Advising:

  • Brokerage Services - Brokerages are an additional choice for case management for adults who live in their own home or with their family. Learn about My Brokerage, My Choice and available providers within Washington County.

Employment:

  • Employment First - A variety of information regarding gaining employment, how to find an employment first team, how to make a difference in your career, finding supported employment services, and so much more.
  • I Work We Succeed - Contains success stories about people seeking employment. Also contains brochures and posters regarding the work being done towards supported employment.
  • Roadmap to Employment - Assisting people with viewing a step-by-step guide to seeking employment.

Guardianship & Supported Decision Making:

Medicaid:

  • Oregon One Eligibility - This links to the ONE Portal where Medicaid account management can take place. You can apply for benefits or receive assistance with current Medicaid benefits.

Money Management:

  • Oregon ABLE Account - Information and resources regarding saving money while employed and/or receiving benefits.
  • Benefits Counseling is available. Certified benefits counselors help people with SSI or SSDI disability benefits understand how working may affect their benefits and how work incentives can help them meet their job goals.
