Programs and Implementation

Supportive Housing Services provides critical resources to people experiencing homelessness to successfully transition into permanently affordable housing using a housing-first model.

To achieve our program goals a broad system of care is being built to create housing opportunity, housing stability, and housing justice throughout Washington County in coordination with our regional partners.
The expanded services offered are delivered through community-based organizations providing culturally specific and trauma informed care to help people end their homelessness as quickly as possible with safe and stable housing.

This system is coordinated though Community Connect, a resource and referral tool that ensures people are connected efficiently and effectively to the housing services that will meet their needs. Through Community Connect, clients are matched with “Housing Case Managers” who support clients throughout their housing journey.

In addition to housing case managers, we administer a variety of shelter options throughout Washington County, provide extensive training to our community partners, and fund outreach providers who meet people experiencing homelessness where they are at to bridge the gap.
