Most parents have questions or needs after bringing their baby home and that is expected. Family Connects registered nurses are here to listen to you.
The no-cost visit includes a health check for mom and baby; tips and answers to your questions; and assistance with connecting you and your family to the resources you need.
If your baby is born at Providence St. Vincent Medical Center or OHSU Health Hillsboro Medical Center, we will call you when your baby is about one week old to schedule a visit.
If your baby is born at another hospital or birthing center, and you live in Washington County, please contact us to schedule a visit. Call 503-846-5300 or complete this referral form.
