Working with a Service Coordinator
After being determined eligible for Intellectual and Developmental Disabilities (I/DD) services with our program, individuals are assigned a Service Coordinator (SC) to provide case management services.
Case Management (Service Coordination) includes the assessment of support needs and desired outcomes, choice advising and planning, monitoring, and referral to community resources.
Service Coordinators advocate for clients and assist as they and their families transition through the stages of development. For individuals enrolled with Washington County Developmental Disabilities Services, having a Service Coordinator (SC) means having a resource for questions and support. Service Coordinators may provide different supports based on your age, and your choice of available services. Washington County DD Services are self-directed and person-centered. Your Service Coordinator is a resource to you.
Our Service Coordinators will support with:
- Conducting choice counseling for individuals to access desired services.
- Creating and coordinating an annual plan that is tailored to individuals' unique needs.
- Facilitating needs assessments with individuals and their families or support persons.
- Completing referrals to requested or needed services and community programs.
- Providing guidance in selecting and accessing appropriate services.
- Identifying and supporting changes in needs.
- Ongoing monitoring of well-being and safety.
- Reviewing satisfaction of support.
