Wraparound Care Coordination

Support is available for youth and their families who have complex needs. Learn about Wraparound and how to access this service.

Service description

Wraparound is a team-based planning process that supports youth and families with complex needs. It brings together friends, relatives, teachers, counselors and caseworkers who help the youth and family reach their goals. The team works with the youth and family to develop an individualized plan. Participation in Wraparound is always voluntary.

This short animated video (less than four minutes) offers a general introduction to Wraparound for children, families and others who may be learning about it for the first time.

Eligibility

Wraparound is available to youth who are on the Oregon Health Plan and assigned to a local coordinated care organization.

To access services, you must meet the following criteria:

  • Be under the age of 21
  • Be struggling in several areas such as home, school, or social relationships
  • Be involved with two or more systems such as Special Education, Child Welfare, Juvenile Services, Mental Health, Developmental Disabilities

To apply for Washington County Wraparound, please complete a referral packet.

Additional information

The Wraparound Review Committee screens referrals. The legal guardian and person making the referral will be notified in writing of the decision. If the youth is eligible for Wraparound, a care coordinator will be assigned. The care coordinator will contact the legal guardian within 14 days. If a youth is not eligible for Wraparound, the Wraparound Review Committee will recommend other resources to the family.
