The 24/7 Washington County Communicable Disease Reporting line is:

All Washington County physicians, health care providers and laboratory workers are required by law to report certain diseases, conditions and outbreaks to Washington County Department of Health and Human Services.

The Oregon Health Authority has also established communicable disease reporting guidelines.

To report suspected or confirmed cases of syphilis, chlamydia, or gonorrhea, health care providers may fill out either of the forms below to the best of their ability. Requested details include patient info, reason(s) for exam, diagnosis and any treatments used.
